Physicals for local high school and middle school athletes will be administered Thursday, April 17 at North Okaloosa Medical Center.
Athletes wishing to participate in sports for the 2014-15 school year can attend as follows:
•Baker School and Laurel Hill School students: 4:45 p.m.
•Crestview High: 5 p.m.
•Davidson Middle and Shoal River Middle students: 5:45 p.m.
Cost is $10 and checks should be made out to Crestview High School.
Forms are available at www.crestviewbulldogs.org/athletic-paperwork. Paperwork must be filled in with a parent's signature before the student can receive the physical.
