Crestview Childcare Network participates in state food program (FORM)

CRESTVIEW — Childcare Network No. 268 in Crestview is a U.S. Department of Agriculture Child Care Food Program participant.

The program reimburses child care providers for serving nutritious meals and snacks to enrolled eligible children at the center, located at 4798 Live Oak Church Road, Crestview.

Parents or guardians of children eligible for free or reduced-price meals must complete an application. Information to determine eligibility is:

●the names of all household members

●household residents' income or Food Assistance (Food Stamp) Program case number, or Temporary Assistance for Needy Families Case number

●signature of an adult household member, and last four digits of that person's Social Security Number or indication that they don't have an SSN

Children of households already receiving FAP or TANF benefits, enrolled in Head Start or Early Head Start programs, and foster children are automatically eligible with appropriate documentation.

Children whose income is at or below the levels show on the income chart below are eligible for free or reduced-price meals.
